Hello,

this patch moves #include isc/sockaddr.h from lib/dns/forward.c to
lib/dns/include/dns/forward.h.

Previously, the header lib/dns/include/dns/forward.h did not work when
included as it was because size of isc_sockaddr_t could not be determined
from included headers.
